Prolonged Delays in Test Results Prove Fatal for Mother of Three
An emotional inquest at Pontypridd Coroners’ Court recently revealed the tragic story of Sarah Catherine Daniels, who faced an untimely demise due to a prolonged delay in crucial pathology test results. The 43-year-old mother of three battled against aggressive cancer but ultimately succumbed to the illness after waiting nine months for results that could have expedited her treatment.
Sarah initially sought medical advice in October 2021 when she began experiencing worrying gynaecological symptoms. Her general practitioner quickly referred her for an ultrasound, which identified a mass in her ovary, leading to a referral within the Cwm Taf Morgannwg University Health Board’s gynaecology department. Presented with options, Sarah chose a procedure that would avoid triggering premature menopause.
Critical Delays Amid Strained Health Services
In February 2022, a less invasive surgery was performed to remove the mass. Although visually it did not appear cancerous, samples were sent for further examination. The pathology department, overwhelmed by significant pressures attributed to the aftermath of the COVID-19 pandemic, experienced significant delays, prolonging diagnosis and treatment initiation.
During the inquest, biomedical scientist Martin Ludlam highlighted how increased workloads and limited capacity affected their ability to manage the demand during 2021 and 2022. As a result, Sarah’s condition advanced unchecked, with her family being unaware of the terminal nature of her illness for far too long.
Unwavering Determination in the Face of Adversity
Despite eventually being informed of her terminal status, Sarah demonstrated relentless determination, seeking private medical interventions at the Christie cancer treatment centre in Manchester. Her efforts included chemotherapy sessions at the Velindre Cancer Centre in Cardiff, which offered temporary hope as she initially received a clean bill of health.
Regrettably, subsequent examinations uncovered the return of cancer, but fluctuating blood markers hindered consistent chemotherapy treatments. Consultant Mohamed Khalifa, who heads a gynaecological cancer team, testified that Sarah lost vital treatment time due to the delays in pathology results and that these delays severely affected her survival chances.
A Call for Improved Health Services
Her husband, Ben Daniels, paid tribute to Sarah’s resilience and commitment to fighting her illness. The coroner, Rachel Knight, candidly criticized the pathology backlog as “unacceptable” and emphasized that improvement is necessary to prevent similar tragedies in the future.
Sarah passed away peacefully in her home in Porth, Rhondda Cynon Taf, surrounded by loved ones, in April 2025. Her story stands as a somber reminder of the human cost of delays in medical testing and the critical need for efficient healthcare services.
